The key to the quality of our service is our unique case management system that includes: • Communication and liaison with GPs, specialists, hospitals and allied health providers • Centralised coordination of care and community resources by Senior Case Managers • 24 hour a day telephone support for patients by these same Case Managers • Constant communication between Home Nurses’ community registered nurses and our Case Managers The goal of Home Nurses Case Management System is to: • Give doctors a high level of control over the care of their patients • Avoid hospitalisation or decrease patients length of stay in hospital • Decrease rates of hospital re-admission from a patient’s primary or secondary diagnosis • Shorten patient recovery time • Refer patient to community resources as appropriate |

Referrals can be made by phoning 8372 4999. A Case Manager will take the referral over the phone. Alternatively you can fax a completed referral form to our office.
